Collection & Request Instructions:

All blood bank specimens have MINIMUM labelling requirements as follows; (a) Sample - Must have the complete surname, firstname and UR number and Date of birth clearly written without error, and signed by the blood collector. Date and time of collection must also be written on the specimen (b) Request slip - All patient details as stated above, required blood product information, date and time of sample collection AND the specimen declaration must be signed by the blood collector. The request slip MUST be sent with the sample. Samples not meeting the above requirements will be discarded WITHOUT EXCEPTION. Clinical notes and past transfusion history must also be noted on the request slip or blood products will not be provided.
